Overview

We are seeking California Licensed Nurse Care Managers to join our Care and Case Management team, focusing on holistic member care through telephonic support and coordination with a diverse clinical team.

In Short

• Deliver patient-centered virtual care management via telephone or video.

Requirements

• Bachelor of Science in Nursing (BSN).
• Current CCM Certification or eligibility for exam in August 2025.
• 5+ years of nursing experience.
• 2+ years of experience in care management.
• Active California Nursing License.
• Must reside in a compact NLC state.
• Experience working remotely and using medical record systems.
• Empathetic approach to patient care.
• Ability to communicate effectively with patients and families.
• Spanish speaking is desirable.
