We are seeking dedicated Nurse Care Managers to join our Care and Case Management team, focusing on holistic member care and guiding them through their healthcare journey.

In Short

• Deliver coordinated, patient-centered virtual Care Management by telephone or video.
• Create impactful care plans with members and a diverse care team.
• Assist members in navigating complex medical conditions and benefits.
• Partner with local providers to ensure coordinated care.
• Provide compassionate, longitudinal follow-up care.
• Support members through acute healthcare episodes.
• Coordinate resources addressing clinical and social problems.

Requirements

• Bachelor of Science in Nursing (BSN).
• Current CCM Certification or eligibility for exam in August 2025.
• 5+ years of nursing experience.
• 2+ years in care management.
• Active Compact RN license.
• Willingness to maintain licensure in multiple states.
• Experience working remotely and using various medical record systems.
• Empathetic and able to communicate effectively with patients.
• Ability to work in a fast-paced environment while maintaining quality.
• Strong writing skills for patient-centric communication.
