In Short

• Manage a panel of complex pediatric patients, providing comprehensive oversight of their care plans.
• Collaborate with pediatric RNs and other care team members.
• Educate and empower parents by providing tailored education.
• Leverage virtual platforms to conduct patient visits.
• Partner with a multidisciplinary team to deliver coordinated care.
• Support new market launches during go-live phases.
• Provide additional support to existing markets as needed.

Requirements

• Minimum 5 years’ experience with a pediatric population required.
• Completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C).
• Hold an unrestricted NP or PA state license in Texas, New York, and Florida.
• Medicaid enrollment in New York and/or Texas is highly preferred.
• Willingness to become cross licensed in new states as requested.
