Outreach Care Assistant - Remote

Overview

The Outreach Care Assistant provides both clinical and clerical support, ensuring quality and compassionate evidence-based care in a virtual Value-Based Care environment for children with special health care needs.

In Short

• Accept incoming phone calls and return calls from caregivers and community partners.
• Pull daily list for TOCs and work closely with the Outreach RNs.
• Create TOC hospital specific list to send daily to hospital partners.
• Place calls and create template-based letters as directed.
• Review schedule weekly to ensure patient lists and PCP reports are pulled.
• Input new endorsement or PCP specific instructions into Care Hub.
• Set up PCP meetings by working closely with clinical teams.
• Explain the Imagine program and enroll caregivers/children if needed.
• Perform other duties as assigned.

Requirements

• High-school level education or equivalency.
• Medical Assistant Certification or equivalent, or at least 2 years clinical experience.
• Spanish fluency preferred.
• Excel experience.
• Eastern timezone preferred.
