Behavioral Health Field Care Manager - Remote

Overview

As a Behavioral Health Field Care Manager, you will assess and evaluate members' needs to help them achieve optimal wellness by guiding them towards appropriate resources and support.

In Short

• Conduct telephonic and face-to-face assessments of members' needs.
• Create and monitor care plans for members.
• Collaborate with providers and community services.
• Identify and resolve barriers to effective care.
• Ensure delivery of needed services for behavioral and physical health.
• Travel within assigned territory in Louisiana.
• Maintain compliance with HIPAA and member confidentiality.
• Utilize Microsoft Office for documentation and communication.
• Participate in a transdisciplinary care team.
• Work remotely with occasional office visits for training.

Requirements

• Active, unrestricted Louisiana license as LCSW, LPC, LMFT, LAC, or RN.
• 2+ years of experience in behavioral health.
• Experience with health promotion and wellness coaching.
• Proficiency in Microsoft Office programs.
• Bilingual in English and Spanish preferred.
• Ability to travel up to 75% within Louisiana.
• Valid driver’s license and reliable transportation.
• Ability to work from a dedicated, interruption-free space.
• Knowledge of community mental health services.
• Experience supporting patients telephonically.
