Field Care Manager, Behavioral Health RN - Remote

Overview

The Field Care Manager, Behavioral Health assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of member.

In Short

• Assess and evaluate member's needs.
• Employ strategies to manage physical and psycho-social health issues.
• Perform telephonic and face-to-face assessments.
• Create member care plans.
• Collaborate with providers and community services.
• Monitor care and ensure progress towards desired outcomes.
• Coordinate with transdisciplinary care team.
• Submit incident reports.
• Travel up to 75% within Louisiana.
• Work from home with specific internet requirements.

Requirements

• Current Unrestricted Registered Nurse License in Louisiana.
• 2+ years of experience in behavioral health.
• Experience with health promotion and coaching.
• Proficiency in Microsoft Office.
• Bilingual in English and Spanish preferred.
• Experience in Community Mental Health preferred.
• Valid driver's license and reliable transportation.
• Ability to work effectively from home.
• Commitment to improving consumer experiences.
• Must reside in Louisiana.
