Become a part of our caring community and help us put health first. The Care Manager, Telephonic Nurse 2 employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues.

In Short

• Work from home as a RN Care Manager.
• Telephonic outreach to assess and support members' health.

Requirements

• Active Registered Nurse (R.N.) license.
• Minimum of 3 years of clinical nursing experience.
• Intermediate to advanced computer skills.
• Effective communication and interpersonal skills.
• Ability to manage multiple priorities simultaneously.
• Must have a separate room for privacy while working.
• Experience with chronic conditions care management preferred.
• Bilingual in English and Spanish is a plus.
• Knowledge of MCG or CMS guidelines is preferred.
• CCM certification is a plus.
