Case Manager - Remote

Overview

Holon Health is seeking a compassionate and highly organized Case Manager to join our collaborative care team. The Case Manager will play a crucial role in proactively monitoring client progress, ensuring adherence to treatment plans, and providing ongoing support to facilitate positive treatment outcomes.

In Short

• Full remote role, must be located in the United States.
• 40 hours per week with some flexibility.
• Monitor client progress and adherence to treatment plans.
• Identify barriers to client engagement and address them.
• Coordinate client appointments and referrals.
• Provide ongoing support and encouragement to clients.
• Strong communication and organizational skills required.
• Bilingual in English and Spanish preferred.
• Experience in a behavioral health setting preferred.
• Compensation includes $23 per hour plus incentives.

• Outstanding communication and interpersonal skills.
• Exceptional organizational and time management skills.
• Understanding of the healthcare system and community served.
• Ability to provide emotional support to clients.
• Strong computer skills, EHR/EMR experience preferred.
• Ability to work independently and flexibly.
• High school graduate or equivalent required; associate degree preferred.
• 1-3 years of experience in a behavioral health setting preferred.
