Admission & Discharge Registered Nurse - Remote

Overview

The Admission and Discharge Nurse is responsible for executing patient admissions and discharges for Help at Home's Home Health patients, balancing home and field work in the Indianapolis territory.

In Short

• Complete initial evaluation visits to determine patient needs.
• Initiate the patient’s plan of care and service plan.
• Complete comprehensive assessments and manage physician orders.
• Collaborate with Client Intake and Recruiting teams for patient admissions.
• Conduct discharge visits as required.
• Ensure regulatory compliance with initial plans of care.
• Assist patients in directing them to appropriate resources.
• Coordinate care with other agencies and providers.
• Attend marketing opportunities to promote Help at Home.
• Counsel patients and families regarding nursing needs.

Requirements

• State of Indiana or multistate RN license.
• Graduate of an accredited nursing school; BSN preferred.
• Home care experience preferred.
• Experience with medically fragile clients.
• Ability to exercise initiative and independent judgment.
• Excellent interpersonal and communication skills.
• Energetic team player with a positive attitude.
