Overview

The Registered Dietitian will serve as an expert in health and nutrition for the Healthmap Solutions Kidney Health Management program, promoting healthy eating habits to assist in managing chronic kidney disease.

In Short

• Promote nutrition and health awareness through individualized therapy and education.
• Provide expert advice on healthy eating habits for chronic kidney disease management.
• Utilize the Nutritional Care Process for evaluating health and nutritional needs.
• Collaborate with Care Navigators for self-management education.
• Encourage adherence to treatment plans.
• Work with outside agencies for continuity of care.
• Participate in team meetings and provide updates.
• Identify barriers to lifestyle changes and monitor progress.
• Document in electronic medical records.
• Perform related duties as assigned.

Requirements

• Degree in foods and nutrition; Master's preferred.
• Active Registered Dietitian license.
• Minimum 5 years of clinical/community experience.
• Experience with chronic kidney disease management required.
• Willingness to obtain additional licenses.
