Care Coordinator - Remote

Overview

The Care Coordinator is responsible for facilitating patient-centered care through the coordination of clinical and non-clinical services in a remote environment. This role ensures continuity of care, supports patient engagement, and assists individuals in navigating healthcare services and resources.

• Coordinate care activities for assigned patients across providers, services, and care settings

Requirements

• Bachelors degree in Nursing, Social Work, Public Health, Health Administration, or a related field
• 3–5 years of experience in care coordination or healthcare support roles
• Strong knowledge of healthcare systems, insurance processes, and care transitions
• Excellent communication, organizational, and problem-solving skills
• Ability to work independently in a fully remote environment
• Proficiency with EHR systems and standard office software
• Reliable internet connection and private remote workspace
