• Collect Health Risk Assessment data both telephonically and face to face.
• Schedule and perform home visits to collect HRA data.
• Communicate patient needs to the Nurse Manager.
• Maintain accurate records of calls and telephonic HRA.
• Requires a current unencumbered State professional nurse license.
• Proficient in basic clinical skills and Microsoft Office.
