Overview

$145.00 per in-home visit (includes mileage) & $85.00 per telehealth visit

PRN NP, 8hrs per week minimum seeing patients in 30 mile radius of home in Calhoun County, MI, for 1x Annual Medicare Wellness Exams and Medicaid Pediatric Chronic Care Confirmation Appointment.

In Short

• Conduct comprehensive in-home health risk assessments
• Perform telehealth visits
• Document patient visits per NCQA standards
• Collaborate with patients, caregivers, and medical staff
• Maintain patient confidentiality
• Participate in professional development activities
• Adapt to varying patient environments
• Handle and lift patients as necessary
• Maintain a valid driver’s license
• Work a minimum of 8 hours per week

Requirements

• Active/unrestricted nurse practitioner license
• Board certification (ANCC, AANP, or NCCPA)
• Active BLS Certification
• Current enrollment in Medicare/Medicaid
• Outstanding EHR skills
• Geriatric training/experience preferred
• Effective communication skills
