In Short

• Conduct comprehensive in-home health risk assessments.
• Take patient history and determine diagnoses.
• Provide documentation of patient visits.
• Communicate with patients and other healthcare providers.
• Perform clinical duties while observing safety protocols.
• Maintain patient confidentiality.

Requirements

• Active nurse practitioner license in coverage states.
• Board certification from recognized nursing organizations.
• Active BLS Certification.
• Current enrollment in Medicare/Medicaid.
• Valid driver’s license and good driving record.
• Outstanding EHR skills.
• Geriatric training/experience preferred.
• Teamwork and effective communication skills.
