In Short

• Deliver annual risk assessment in a residential setting or telehealth.
• Conduct comprehensive in-home health risk assessments.
• Document patient visits per NCQA standards.
• Communicate with patients, caregivers, and other providers.
• Maintain patient confidentiality and observe OSHA precautions.
• Participate in professional development activities.
• Work collaboratively with medical staff and patients.
• Adapt to differing patient environments and conditions.
• Lift and carry items weighing up to 50 pounds.
• Drive up to 150 miles daily as needed.

Requirements

• Active/unrestricted medical license.
• Active CPR Certification.
• Board eligible or board certified in family medicine.
• Outstanding EHR skills.
• Geriatric training/experience preferred.
• Effective teamwork and communication skills.
