In Short

• Collect Health Risk Assessment data telephonically and face to face.
• Schedule and perform home visits for data collection.
• Communicate patient needs promptly to Nurse Manager.
• Maintain accurate records of calls and assessments.

Requirements

• A current unencumbered State professional nurse license.
• Proficient in basic clinical skills.
• Proficient in Microsoft Office.
