• Conduct health risk assessments in-home or via telehealth.
• Perform patient evaluations and document visits.
• Communicate with healthcare teams and patients.
• Maintain patient confidentiality and follow safety protocols.
• Participate in ongoing professional development.
• Must have active nurse practitioner license and certifications.
• Geriatric experience is preferred.
• Work adaptable to various environments.
• Ability to lift and transport patients if necessary.
• Drive to patient locations as needed.

Requirements

• Active/unrestricted nurse practitioner license.
• Board certification (ANCC, AANP, or NCCPA).
• Current CPR certification.
• Enrollment in Medicare/Medicaid.
• Valid driver’s license and good driving record.
• Outstanding EHR skills.
• Geriatric training/experience preferred.
• Effective teamwork and communication skills.
