Overview

The RN Case Manager at Guidehealth provides remote support to patients, helping them navigate their healthcare needs through telephonic assessments and care plan development.

In Short

• Conduct telephonic assessments to understand patients' needs.

Requirements

• Active Compact Registered Nurse license.
• 3+ years of RN Case Management experience.
• Strong critical-thinking and problem-solving skills.
• Ability to work independently and manage priorities.
• Excellent communication and telephonic interviewing skills.
• High comfort level with technology and EMRs.
• Understanding of quality standards (NCQA, HEDIS).
• A quiet home office suitable for patient calls.
• Alignment with clinical protocols and compliance standards.
• Preferred: BSN and Case Management certification.
