Overview

The RN Case Manager at Guidehealth is responsible for providing compassionate, person-centered support to patients remotely, helping them navigate their healthcare needs.

In Short

  • Conduct telephonic assessments to understand patient needs.

  • Complete Medicare Annual Wellness Visits via telehealth.

  • Document accurately in EHR systems.
  • Participate in virtual meetings and ongoing education.

Requirements

  • Active Compact RN license in good standing.
  • 3+ years of RN Case Management experience.
  • Strong critical-thinking and problem-solving skills.
  • Ability to work independently and manage multiple priorities.
  • Outstanding communication skills.
  • Excellent telephonic interviewing skills.
  • High comfort level with technology.
  • Understanding of accreditation and quality standards.
  • A quiet home office suitable for patient calls.
  • Alignment with clinical protocols and compliance standards.
