Overview

As a remote RN Case Manager at Guidehealth, you will enhance the quality of member management and assist in navigating the healthcare system as part of a collaborative health team.

In Short

  • Work remotely as part of the Value Based Care Services team
  • Conduct telephonic outreach to members and providers
  • Analyze data for member eligibility in health programs
  • Coordinate and provide member-centric care
  • Document and report in compliance with standards
  • Assist members in achieving health goals
  • Participate in quality management committee meetings
  • Maintain confidentiality of medical records
  • Engage in continuous professional development
  • Handle off-hour/weekend calls as needed

Requirements

  • Current IL Registered Nurse License
  • Minimum of five years of healthcare experience
  • Experience in Case Management and Chronic Condition Management
  • Knowledge of managed care and community health
  • Previous remote work experience preferred
  • Strong communication and organizational skills
  • Ability to work independently and manage multiple tasks
  • Proficient in MS Office and EMR systems
  • High-speed internet and secure home office required
  • Certification in Case Management is a plus
