Overview

Join our caring community at Humana Gold Plus Integrated as a Long-Term Services and Support (LTSS) Care Coordinator, where you will assess and evaluate members’ needs to help them achieve optimal wellness.

In Short

• Assess and evaluate members’ health needs.
• Conduct home visits to Medicaid members.
• Coordinate services and monitor care plans.
• Identify and resolve barriers to effective care.
• Guide members towards appropriate resources.
• Utilize electronic medical records and software.
• Communicate effectively with members and stakeholders.
• Travel 75-90% for field interactions.
• Work Monday to Friday with flexible hours.
• Participate in Humana’s TB screening program.

Requirements

• Bachelor’s degree in social sciences or related field, or LPN license in Illinois.
• 1+ years of experience in healthcare or case management.
• Experience with Medicare and Medicaid recipients.
• Intermediate to advanced computer skills.
• Exceptional communication and interpersonal skills.
• Ability to travel for face-to-face meetings.
• Valid state driver's license required.
• Bilingual skills preferred.
