• Manage a CCM caseload and carry out individualized nephrology-focused care plans.
• Collaborate with physicians, APPs, and care teams to ensure continuity of care.
• Educate patients on disease management, treatment goals, and self-care strategies.
• Coordinate referrals and community resources to remove barriers to care.
• Use telephonic and virtual tools to engage patients and document care activities.
• Remote flexibility may be offered based on experience and demonstrated proficiency.
• Ideal for RNs with prior CCM, home health, hospice, or nephrology experience.
• Comfortable using video/audio communication tools for virtual patient interaction.
• Proficiency in EMR systems (EPIC a plus).
• Work independently and manage a caseload effectively.

Requirements

• RN licensed in NC or SC.
• 2+ years in chronic care coordination, case management, or nephrology strongly preferred.
• Excellent communication, clinical judgment, and follow-through.
• Proficiency in EMR systems (EPIC a plus).
• Comfortable using video/audio communication tools for virtual patient interaction.
