Supportive Care Case Manager (RN) - Remote

Overview

As a Supportive Care Case Manager (RN) at DaVita Integrated Kidney Care, you will assist complex patients in navigating the healthcare system, focusing on their medical, social, emotional, and financial needs.

In Short

• Full-time remote position.
• Support patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD).
• Coordinate care with healthcare team members.
• Develop and implement care plans aligned with patient wishes.
• Facilitate communication between patients and families.
• Provide comfort care guidance for patients.
• Build empathetic relationships with patients.
• Manage care transitions and ongoing patient needs.
• Assist in developing advance care plans.
• Work Monday to Friday, accommodating patient availability.

Requirements

• 2+ years of RN experience in Hospice or Palliative medicine.
• 5+ years of RN experience required.
• Current RN License is required, BSN preferred.
• CPR or BLS certification required.
• Intermediate computer skills, proficiency in MS Word, Excel & Outlook.
• Home office with internet connectivity required.
• IKC experience preferred.
• 2+ years of Case Management or Chronic Care Management experience preferred.
• Spanish-speaking preferred.
• Compact License preferred.
