Bilingual Specialty Health Coach - Diabetes - Remote

Overview

As a Bilingual Specialty Health Coach-Diabetes at DarioHealth, you will support, encourage, and educate members with chronic conditions as they work towards reaching their health goals.

In Short

• Coach a caseload of Dario members across all Dario conditions using multi-modal communication.
• Serve as coaching team SME for Diabetes, providing additional support related to Diabetes condition.
• Provide training and assistance to members on using their Dario device(s) and navigating the DarioHealth app.
• Address Social Determinants of Health (SDoH) needs and provide resources to members.
• Support members in creating and sustaining new habits.
• Be member-centric, meeting members where they are in the process.
• Understand and value the coaching scope of practice in chronic disease management.
• Other duties as assigned.

Requirements

• CDE, RN or Clinical License.
• Bachelor’s degree or higher.
• 5+ years of experience in health coaching, health education, nutrition, or behavioral health.
• Familiarity with motivational interviewing and behavior change theories.
• Open-minded, non-judgmental, and compassionate.
• Health Coach Certification preferred.
• Bilingual in English/Spanish preferred.
• Demonstrated effective organizational and communication skills.
• Independent, creative self-starter, flexible, and thrive in a fast-paced environment.
• Ability to learn and convey complex information.
