Case Manager RN - Remote

Overview

The Case Manager RN at CVS Health will coordinate and facilitate comprehensive care for individuals by assessing their needs and developing personalized care plans.

In Short

• Acts as a liaison with various stakeholders including patients, providers, and insurance companies.
• Implements and coordinates case management activities for catastrophic and chronically ill members.
• Interacts with members telephonically or in person, including home visits.
• Develops plans of care to optimize wellness and medical outcomes.
• Prepares documentation of case work activities.
• Monitors member progress toward desired outcomes.
• Facilitates condition management and return to work.
• Applies relevant laws and regulations in service provision.

Requirements

• Active and unrestricted compact RN licensure.
• 3 years of clinical practical experience in relevant areas.
• 2 years of case management or home health care coordination experience.
• Excellent analytical and problem-solving skills.
• Effective communication and organizational skills.
• Ability to work independently.
• Preferred: Certified Case Manager and additional relevant certifications.
• Efficient computer skills.
• Willingness to obtain multi-state RN licenses if needed.
• Bachelor's of Science in Nursing required.
