Lead Field Nurse (LPN) - Remote

Overview

The Lead Field Nurse (LPN) is responsible for mentoring and coaching field staff while managing patient care through home visits and care coordination activities.

Requirements

• Active LPN, LVN, or RN license.
• At least 2 years of direct patient care experience.
• IV or blood draw certification.
• Current BLS certification.
• Experience in in-home clinical care.
• Proficiency in various clinical procedures.
• Experience mentoring or coaching preferred.
• Training in complex care or geriatrics is a plus.
