Field Nurse (LVN) - Remote

Overview

The Field Nurse (LVN) provides clinical care to patients in their homes and supports virtual clinical care. This role is essential in ensuring patients receive appropriate care in a timely manner.

• Deliver direct nursing care through in-home and virtual visits.
• Facilitate virtual visits and coordinate patient care.

Requirements

• Active LVN license in California.
• 2+ years of clinical experience required.
• IV or blood draw certification.
• Current Basic Life Support (BLS) certification.
• Ability to conduct independent patient home visits.
• Travel 75%+ within the assigned area.
• Experience with aging adults preferred.
• Proficient in computer and software skills.
• Valid driver's license with no recent violations.
• Willingness to be vaccinated against COVID-19.
