Field Nurse (LPN) - Remote

Overview

As a Field Nurse, you will support a panel of patients through a mix of routine and urgent home visits, combined with panel support and care coordination activities.

Requirements

• Active LPN or LVN license required.
• Ability to conduct independent home visits.
• Willingness to travel 75%+ of the time.
• IV or blood draw certification required.
• Current certification in Basic Life Support (BLS) required.
• 1+ years nursing experience required.
• Competency in clinical procedures required.
• Preferred experience in urgent/community home settings.
• Preferred training in complex care or geriatrics.
• Average to advanced computer skills required.
