Overview

The Enrollment Specialist contacts patients to explain our Chronic Care Management Program and offer them the opportunity to enroll. In addition, the Enrollment Specialist gains consent to enroll and manages the enrollment process.

In Short

• Educating potential program participants on the benefits of the Chronic Care Management program
• Retrieving patient information to determine suitability for the program
• Interpreting eligibility requirements and program details
• Protecting patient information security
• Complying with HIPAA and Medicare regulations
• Maintaining knowledge of Medicare Part A and B
• Meeting daily enrollment KPIs
• Other duties as assigned
