Overview

The Senior Care Manager (RN) is responsible for assessing, planning, and implementing complex care management activities to ensure quality healthcare outcomes for members.

In Short

  • Assesses and develops personalized care plans for members.

Requirements

  • Degree from an Accredited School of Nursing or Bachelor's degree in Nursing.
  • 4 – 6 years of related experience.
  • Registered Nurse (RN) State Licensure required.
  • Strong communication and collaboration skills.
  • Ability to develop and assess care plans.
  • Experience in care management or healthcare coordination.
  • Knowledge of regulatory compliance standards.
  • Ability to work flexibly in remote, hybrid, or office settings.
  • Experience with patient education and support.
  • Willingness to engage in continuous learning and improvement.
