As a Health Advocate at Careway, you will be the frontline support for Medicare patients navigating a complex healthcare system, providing personalized support to ensure patients achieve their healthcare goals.

Requirements

• Experience in patient advocacy, care coordination, or related field.
• Understanding of Social Determinants of Health and experience with diverse populations.

• Strong organizational abilities and clear communication.
• Tech-savvy with modern EHR systems and patient engagement apps.
