Overview

As an RN Health Advocate at Careway, you will support seniors in navigating complex health and social challenges, ensuring they receive the care and resources they need.

In Short

• Serve as a frontline support for seniors navigating healthcare issues.
• Address social determinants of health and coordinate resources.
• Build trusting relationships with patients and healthcare providers.

Requirements

• Active Compact or California RN license in good standing.
• Deep understanding of social determinants of health.
• Exceptional active listening and interpersonal skills.
• Strong organizational and communication abilities.
• Comfortable with technology and learning new systems.
• Resilience and creativity in problem-solving.
• Background in community health or health coaching is a plus.
• Fluency in Spanish or other languages is preferred.
