Requirements
Active healthcare license (LCSW, LPC, LMFT) in good standing.
Experience in care management or patient advocacy.
Understanding of Social Determinants of Health.
Strong clinical and organizational skills.
Ability to manage multiple tasks under pressure.
Technical savvy and quick learning of new systems.
Strong bias toward action and execution.
Willingness to provide constructive feedback.
