As an LCSW Healthcare Advocate for Solace, you will work with Medicare patients throughout their healthcare journey, navigating complex health concerns to help them achieve their health and wellness goals.

Requirements

• Active LCSW license in good standing.
• Experience in care management or patient advocacy.
• Understanding of Social Determinants of Health.

• Action-oriented with a bias toward execution.
• Willingness to provide constructive feedback.
