Health Coach, Medical Assistant - Remote

Overview

The Cadence Health team seeks a Health Coach, Medical Assistant to help provide patient care remotely alongside our Registered Nurses and Nurse Practitioners.

In Short

• Support delivery of health care to patients by performing a variety of activities and procedures.
• Provide one-on-one coaching and support to patients managing chronic conditions.
• Conduct comprehensive assessments of patients' health status and lifestyle behaviors.
• Help patients execute on their personalized care plans and goals.
• Monitor patients' progress and health outcomes through regular check-ins.
• Educate patients on disease management and prevention strategies.

Requirements

• Active Certification as a Medical Assistant from an accredited association.
• 5+ years of experience as a Medical Assistant.
• Stable wifi with strong upload and download speeds for remote work.
• Highly skilled in behavioral-based coaching.
• Excellent clinical acumen and competency in patient assessment.
• Exceptional written, verbal, and interpersonal communication skills.
• Prior experience in a high-growth, fast-paced startup environment.
• Experience in patient education for chronic conditions.
• Tech-savvy and capable of working with multiple systems.
