Overview

The Patient-Centered Medical Home (PCMH) Care Coordinator III is a remote-based nurse who collaborates with healthcare professionals to ensure appropriate care delivery for assigned patients.

Requirements

• Associates Degree in Nursing required.
• Current RN license in South Carolina.

• Experience with electronic medical records.
• Strong organizational and communication skills.
• Ability to analyze and interpret data.
• Ability to work independently with supervision.
• Willingness to travel to clinical locations.
• Updated BLS certification.
