Overview

As a Facilitated Telehealth Coordinator, you will serve as the in-person liaison between the patient and our virtual care team, contributing to the advancement of healthcare technology and improving access to care for patients.

Requirements

• Experience in a healthcare setting.

• Excellent communication skills.
• Ability to work independently.
• Driver’s license and car required.
• Bilingual (Spanish-English) preferred.
• Willingness to travel up to 100 miles round trip.
• Experience with mental health conditions preferred.
