Overview

The Program Specialist will contribute to the operational excellence and strategic growth of ACO REACH initiatives, managing core program processes and collaborating with teams to enhance engagement and outcomes.

In Short

• Support day-to-day ACO REACH processes and operations.
• Manage Zendesk communications for the ACO REACH team.
• Utilize Monday.com for process management.
• Generate actionable reports using Medicare data.
• Draft and maintain the ACO REACH newsletter.
• Plan and organize based on the ACO REACH calendar.
• Contribute to interdepartmental projects and meetings.
• Communicate statuses and keep stakeholders informed.
• Assist in collecting SDOH data for CMS.
• Support practice needs related to ACO REACH operations.

Requirements

• Bachelor’s degree in a relevant field.
• 3+ years of progressive healthcare experience.
• Experience with project management tools.
• Data analytics experience using Excel or Google Sheets.
• Willingness to travel as needed.
