Overview

The Health Coach Consultant position involves working as a telephonic case manager with patients and their care teams to enhance medical appropriateness and quality of care, applying clinical judgment to address health needs.

Requirements

• 5+ years of experience as a Registered Nurse.
• 1+ years of Nursing Management experience.
• Experience with patients having Diabetes, Cardiac Disease, etc.
• Good typing skills and proficiency in Microsoft Excel.
• Active and unrestricted RN licensure required.
• Compact Licensure RN License is necessary.
• Experience in Home Health or Case Management preferred.
• Bilingual in Spanish is a plus.
• High School Diploma/Associate degree required; Bachelor’s preferred.
• Motivational Interviewing experience is a plus.
