• Conduct in-home wellness risk adjustment assessments.
• Assess overall health and well-being of member beneficiaries.
• Perform annual in-home wellness visits and risk adjustment health assessments.
• Deliver patient health education on various topics.
• Build relationships of trust with members.
• Assist in closing quality care gaps.
• Collaborate to enhance care delivery.
• Other duties as assigned.

Requirements

• Experience as a Nurse Practitioner.
• Strong communication and empathy skills.
• Ability to perform comprehensive health assessments.
• Knowledge of Medicare and risk adjustment coding.
• Experience with chronic conditions.
