• Pine Park Health is looking for part time or full time Nurse Practitioners.
• Responsible for delivering care in the Fremont, California market.
• Work with a smaller patient panel of complex care patients.
• Contribute to a value-based care model.
• Provide onsite primary care services to older adults.
• Develop and execute person-centered care plans.
• Collaborate with a multidisciplinary team.
• Flexible work schedule with travel within the Fremont area.
• Strong orientation towards collaboration.

Requirements

• Nurse Practitioner with geriatrics experience preferred.
• Ability to work in a fast-paced environment.
• Willingness to provide onsite clinical care.
• Desire to support a holistic care model.
• Strong written and verbal communication skills.
• Ability to work with tech-enabled tools.
