Remote Clinical Care Liaison (Weekends)

Overview

The Clinical Care Liaison is the operational and relational backbone of our geriatric primary care team, managing care coordination for a panel of 500–600 patients.

In Short

• Full-time, 40 hours per week including weekends.
• Primary point of contact for patients, families, and facility partners.
• Triage urgent care requests and maintain patient documentation.
• Manage appointment scheduling and follow-up care.
• Participate in after-hours on-call rotation.
• Contribute to quality improvement initiatives.
• Must have a Bachelor’s degree in a related field.
• 3+ years of healthcare coordination experience required.
• Proficient in EMR/EHR systems.
• Exceptional communication skills needed.

Requirements

• Bachelor's degree required; preferred in Healthcare Administration or related field.
• 3+ years of experience in healthcare coordination or clinical operations.
• Experience with geriatric or value-based care populations preferred.
• Ability to manage competing priorities in a high-volume environment.
• Proficiency with healthcare technology platforms.
• Strong written and verbal communication skills.
• Self-directed and accountable in a remote setting.
• Reliable high-speed internet and a dedicated workspace required.
• Must not be on any exclusion from Federal Health Care Programs.
