Acentra Health is seeking a Care Coordinator (Provider Enrollment) to provide telephonic care coordination outreach activities to support health management programs.

Requirements

• Associate’s degree in health-related field or medical assistant training.
• 4 years of experience in care coordination.
• 2 years of experience in a clinical environment.
• 1 year of telephonic outreach or call center experience.
• Knowledge of medical terminology is preferred.
• Bilingual candidates are preferred.
