Overview

The Resource Specialist at Brigham and Women's Faulkner Hospital plays a crucial role in ensuring patients receive the necessary resources and services for a successful transition to community settings.

In Short

  • Work with care team staff to assist patients in resource management.
  • Develop education and referral information regarding extended care facilities.

Requirements

  • High School Diploma or Equivalent required.
  • Associate's Degree or Bachelor's Degree preferred.
  • Healthcare experience in extended care facilities preferred.
  • Knowledge of medical terminology preferred.
  • Experience in hospital discharge planning is a plus.
  • Ability to work independently and proactively.
  • Basic typing and computer data entry skills required.
  • Extensive knowledge of community organizations and healthcare regulations.
  • Strong interpersonal skills for effective communication.
  • Ability to manage changing priorities effectively.
