Overview

The Care Coordinator will work as part of an interdisciplinary care team providing care management for Dual Eligible Special Needs Plan (D-SNP) members, addressing their medical, behavioral, and social needs.

Requirements

• Bachelor's Degree
• 1+ years of direct clinical experience
• Valid Driver's License and reliable transportation
• Experience with Dual Eligible Populations preferred
• Exceptional communication and interpersonal skills
• Strong organizational skills
• Ability to work independently and in teams
• Knowledge of healthcare and community services
• Competency in health care computer platforms
• Ability to adapt to change
