Overview

The Patient Engagement Specialist will be an integral part of the Population Health Team, responsible for conducting patient outreach to foster engagement in our care model.

In Short

  • Conduct patient outreach through telephonic and in-person visits.
  • Engage with patients who have not visited the clinic.
  • Document interactions in electronic medical records.
  • Communicate complex topics with patients and their caregivers.
  • Partner with Community Health Workers and clinicians.
  • Educate patients about services offered by Oak Street Health.
  • Travel to various patient-centered locations.
  • Respond to escalations and urgent needs.
  • Maintain HIPAA standards and confidentiality.
  • Foster relationships with patients and their support persons.

Requirements

  • Experience in healthcare engagement, sales, or marketing.
  • Strong verbal and written communication skills.
  • Proficiency in technology and EHR/EMR platforms.
  • Ability to operate independently in a fast-paced environment.
  • Understanding of Medicare and Medicare Advantage programs.
  • Access to reliable transportation.
  • Fluency in Spanish preferred.
  • US Work Authorization required.
  • Dedication to community service.
  • Ability to multitask and prioritize work.
