Nurse Practitioner, Advanced Practice Provider - Remote

Overview

As a Nurse Practitioner at Oak Street Health, you will provide effective and equitable value-based primary care to adults on medicare, ensuring they remain happy, healthy, and out of the hospital.

In Short

• Provide exceptional primary care and conduct office visits for routine and acute issues.
• Administer annual wellness visits and health risk assessments.
• Coordinate care with other providers, specialists, and agencies.
• Lead population health initiatives in collaboration with the Care Team.
• Educate patients about their health conditions and care plans.
• Participate in promotional activities for Oak Street Health.
• Conduct home visits as needed.
• Work closely with operational leadership and the Center Medical Director.
• Utilize a holistic view of health in patient assessments.
• Collaborate with a dedicated medical team including medical assistants and scribes.

Requirements

• Master of Science in Nursing or equivalent.
• National certification in a nursing specialty.
• Active state Nurse Practitioner license.
• Active DEA license.
• Experience in primary care is preferred.
• Understanding of HCC documentation and ICD-10 coding.
• Desire for teamwork and collaboration.
• Bilingual proficiency is a plus.
