Overview

As a Nurse Practitioner at Oak Street Health, you will provide effective and equitable value-based primary care to adults on Medicare, ensuring they remain happy, healthy, and out of the hospital.

• Provide exceptional primary care to Medicare patients.

Requirements

• Master of Science in Nursing or equivalent.
• National certification as a Nurse Practitioner.
• Active state Nurse Practitioner license.
• Active DEA license.
• US Work Authorization.
• Experience in primary care preferred.
• Understanding of HCC documentation and ICD-10 coding.
• Teamwork and collaboration skills.
• Bilingual proficiency is a plus.
