Overview

Requirements

• Active Registered Nurse license in Illinois.
• 3-5 years of clinical experience in relevant areas.
• 2-3 years of experience in case management or home health care coordination.
• Strong analytical and problem-solving skills.
• Excellent communication and organizational skills.
• Proficient in Microsoft Office Suite.
• Ability to work independently from home.
• Reliable transportation for travel.
• Preferred: Certified Case Manager certification.
• Preferred: Additional national professional certifications.
