Overview

The Clinical Pharmacist plays a pivotal role in the healthcare team, providing essential pharmacy services that contribute to optimal patient outcomes.

In Short

• Oversee therapy management in accordance with organization policies and protocols.
• Identify and address medication errors and adverse reactions.
• Participate in quality improvement initiatives.
• Ensure adherence to established standards and client policies.
• Provide appropriate medication therapy and address patient concerns confidentially.
• Support medication use processes to ensure safety, efficacy, and compliance.
• Maintain accurate records of clinical interventions and quality improvement activities.
• Work with interdisciplinary teams to support effective patient care.
• Respond to evolving departmental needs and participate in various projects.
• Contribute to the achievement of Quality Improvement goals.

Requirements

• Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy.
• Preferred: Successful completion of an ASHP accredited PGY1 or specialty residency.
• Required: Current Pharmacist license obtained within 2 months of hire date.
• Preferred: 3 years of retail or specialty pharmacy experience.
• American Heart Association BLS certification obtained within 6 months of hire date.
• Strong understanding of pathophysiology and pharmacotherapeutics.
• Ability to interpret and apply regulatory standards.
• Excellent presentation skills and ability to communicate effectively.
• Demonstration of good judgement and multi-tasking abilities.
• Proven ability to diagnose and resolve issues.
